NURSE ASSESSMENT COORDINATOR

(RNAC) $5,000 Sign on Bonus Hybrid Position - Scenery Hills Rehabilitation & Healthcare Center & Havencrest Rehabilitation & Healthcare Center Scenery Hills Rehabilitation & Healthcare Center and Havencrest Rehabilitation & Healthcare Center are seeking an experienced and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to join our team. This is a unique hybrid RNAC position supporting both facilities , offering the opportunity to work collaboratively with two experienced leadership and clinical teams while maintaining flexibility between locations. Position Overview The RNAC will oversee and coordinate the MDS assessment and care planning process , ensuring accuracy, timeliness, regulatory compliance, and appropriate reimbursement. The position will support both Scenery Hills and Havencrest , with on-site presence coordinated based on the operational needs of each facility. RNAC Responsibilities Coordinate and complete MDS assessments accurately and within required timeframes Manage the RAI/MDS process in accordance with CMS and regulatory requirements Monitor resident assessments and ensure appropriate documentation supports MDS coding Coordinate interdisciplinary care planning Review clinical documentation for accuracy and completeness Collaborate with nursing, therapy, social services, dietary, and other departments Monitor reimbursement and case-mix opportunities Participate in Medicare and utilization review processes Maintain compliance with federal, state, and facility requirements Educate and support clinical staff regarding MDS documentation and requirements Assist with audits and survey preparation Maintain organized and accurate resident assessment records Qualifications Current Pennsylvania Registered Nurse (RN) license in good standing Previous

RNAC/MDS

experience in a skilled nursing facility strongly preferred Knowledge of MDS, RAI, Medicare, and skilled nursing regulations Strong organizational and time-management skills Excellent communication and leadership abilities Ability to collaborate effectively with multiple departments and facility leadership Comfortable working between two facilities as part of a hybrid role Why Join Our Team?
