The responsibility of the Clinical Coordinator will be to plan, develop, and monitor treatment and behavioral support service delivery to meet the needs of individuals diagnosed with various health-related disorders and disabilities. The Clinical Coordinator also consults with and teaches direct service professionals and management staff and collaborates with team members, related service providers, families, and other agencies as appropriate.
Minimum Qualifications:
License Required (LICSW, LMHC, LMFT, LCSW) Master's degree in Social Work, Counseling, Psychology, or a closely related clinical or human services field required. 2-4 years of experience working with people with disabilities and/or elders 1-3 years of experience in leadership with program management and supervision strongly preferred Safe driving record, current driver's license, personal vehicle, and current motor vehicle liability insurance required Ability to successfully pass all background checks. Job Responsibilities
•Responsibilities are based upon the needs of the individual supported and may vary depending on their unique needs and goals
• Demonstrate and model effective techniques for staff or provide direct intervention for individuals when necessary. Train and instruct workers on the proper implementation of treatment interventions. Analyze treatment data to assess the effectiveness of interventions and make data-driven decisions. Provide support, coaching, and performance feedback to assigned case management and/or clinical staff. Assist in developing and revising Treatment Plans, including writing clear and measurable goals and objectives. Collaborate and communicate with organizational clinical staff to provide quality clinical support and also with various stakeholders, BHDDH, and families. Provide in-person consultations and oversight at residential programs. Conduct functional assessments to identify individual needs and tailor treatment strategies accordingly. Take responsibility for the development of Treatment Plans and the writing of goals and objectives for participants receiving services within the agency. Actively participate in organizational quality initiatives, attending staff meetings and contributing to continuous quality improvements and team processes. Ensure compliance with all safety programs and policies to maintain a secure work environment for staff and clients. Serve as a point of contact for staff regarding complex cases, service barriers, crisis situations, family or guardian concerns, and community resource needs. Maintain effective communication with the Vice President of Services and the entire clinical and management team, reporting all pertinent issues related to adult services programs and service delivery.
