We are seeking a compassionate and highly organized Licensed Vocational Nurse (LVN) Patient Care Coordinator to join our team and play a vital role in supporting patient care operations. Position Summary The LVN Patient Care Coordinator serves as the communication and coordination hub for our clinical team. This role works closely with nurses, aides, social workers, chaplains, leadership, patients, and families to ensure timely visits, accurate documentation, regulatory compliance, and exceptional service. The ideal candidate is a strong communicator who thrives in a fast-paced environment, builds trusting relationships with field staff, and consistently demonstrates empathy, integrity, and a servant-hearted attitude.
