Job Description

Company Overview:

Responsibilities:
Reviews available patient information related to the case, including home visits, to determine the appropriate needs of the patient. Assigned appropriate personnel to the case as needed. Confers with Medical Director/patient physician regarding any questions and an individual's eligibility for services. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and by services provided. Instructs and guides clinicians to promote more effective performance and delivery of quality care. Must always be readily available to clinicians during operating hours. Assists clinicians in establishing immediate and long-term patient goals that align with the patient's plan of care. Attends case conference meetings with personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. Conducts record reviews and communicates findings and recommendations to the Senior leadership team. Assists in the screening and interview process of new employees when designated and makes recommendations for employment of individuals. Assists in the orientation of new employees. Assists the Administrator, and Alternate Administrator, and other Solaris management in the planning, implementation and evaluation of in-service and continuing education programs. Evaluates and counsels assigned staff as needed. Actively participates in quality assessment performance improvement teams and activities. Other duties as assigned by management. Adheres to the Standards of Conduct.
Qualifications and Requirements:
Active Registered Nurse license, in good standing.
Preferred:
2 years Home Health management experience. Proven ability to work within an interdisciplinary setting. Must have a current drivers license, in good standing. Demonstrates excellent observation, verbal and written communication skills. Excellent leadership, communication and interpersonal skills. Strong knowledge of healthcare regulations, standards and best practices.
