Care Coordinator

RESPONSIBILITIES
A Care Coordinator is a vital member of Teresa's House healthcare team as they are responsible for managing and organizing resident information and assisting the DON to ensure smooth and efficient day-to-day operations. Care Coordinators are responsible for maintaining accurate and up-to-date resident records, scheduling appointments and communicating with healthcare professionals. They are also responsible for ensuring that all necessary clinical supplies and equipment are readily available. In addition to these administrative tasks, Care Coordinators also play an important role in the overall resident experience. They are often the first point of contact for residents and their families and are responsible for providing information and answering questions. Care Coordinators must have strong organizational skills, attention to detail, and excellent communication and interpersonal skills. They must also be comfortable working in a fast-paced, high-pressure environment and be able to work well under pressure.
QUALIFICATIONS
; 1. Prefer CMA or healthcare experience equivalent to CMA. 2. Be in good standing on Nurse Aide Registry and be able to fulfill all qualifications of a Nursing Assistant. 3. Preferred one to two years of experience in either Assisted Living or Long Term Care. 4. Ability to understand, remember and carry out oral and written instructions in English. 5. Ability to communicate effectively both verbally and in written format with residents, families, other care providers and supervisors. 6. Excellent computer skills, ability to utilize electronic documentation. 7. Must be able to relate positively and favorably to residents and families and to work cooperatively with other employees. 8. Must be capable of maintaining regular attendance. 9. Knowledgeable of Teresa's House Mission, Vision and Values. Conducts self in a manager that is congruent with the Mission, Vison and Values of Teresa's House. 10. Ability to safely care for residents in a patient and respectful manner. 11. Ability to complete assignments while having constant interruptions. 12. Knowledge of Resident Rights per Assisted Living regulations. 13. Must be able to identify issues to be confronted or otherwise resolved pertaining to the dynamics of residency. 14. Must be capable of performing the Essential Job Functions on this job, with or without reasonable accommodation.
ESSENTIAL FUNCTIONS
1. Provide excellent customer service and professionalism and lead by example; i.e., pitch in whenever necessary for any tasks. 2. Assist DON with new and return admissions to Teresa's House. Greet resident and family and ensure they feel welcomed. Assist with admission per DON direction. 3. Distribute and collect clinical and non-clinical forms per policy: Daily assignment sheets with staff signature, daily temperature logs, food temperature logs, shower sheets, assignment sheets, meal preferences, emergency preparedness forms,,, 4. File MD orders after being reviewed by DON. 5. Assist with staffing call-ins while on duty when staffing coordinator is off. 6. Inspect medication carts for expired medications, dated open vials, non-oral medications in plastic bags for infection control purposes… 7. Assist DON with obtaining psychotropic consents per regulations. 8. Assist DON with monitoring new employee/annual TB screening. Schedule staff for second TB screening and all follow-ups. 9. Arrange service plan meetings with families per DON request. 10. Conduct chart audits per assignment. 11. Conducts nursing supply inventory weekly. Maintains a par level and provides DON with weekly list to ensure all supplies are available to meet resident's needs. Provide data on resident incontinent brief sizes. 12. Responsible for obtaining resident weights and providing DON with reports. Follow-up per DON direction. 13. Assist DON with screening and interviewing new applicants 14. Assist residents with eating as needed per DON request. Make observations Oversees the dining room process - ensures all residents have appropriate utensils and other supportive devices to assist in eating 15. Assists DON with monitoring Home Health and Hospice visit forms. Pick up forms, after DON review, file forms in resident's charts. 16. Other tasks as assigned. 17. Encouraging residents to maintain their highest possible independence, while maintaining their resident's rights. 18. Participate in teaching and promoting self-care for residents. 19. Participate in emergency fire or disaster drills and other in-service training as scheduled. 20. Participate in departmental meetings and family meetings as directed. 21. Follow Universal Safety Precautions.
