Key responsibilities Order management:
Receive, document, and verify physician orders for patient care services. Track the progress of orders from initial referral to service initiation. Follow up on verbal orders to ensure they are accurately documented and implemented.
Care coordination:
Coordinate with physicians, case managers, and other clinical staff to implement care plans. Liaise with referral sources to ensure the agency receives all necessary information. Ensure timely initiation of services by contacting the client and scheduling the start of care within the required timeframe.
Documentation and reporting:
Maintain accurate records of patient care, progress, and any issues. Ensure all documentation is completed in accordance with regulatory requirements. Report any issues that could impact the care plan, such as missed visits, to the appropriate personnel.
Problem-solving:
Identify and address barriers or problems in the system that could delay or prevent care. Support the care team by providing administrative assistance to ensure smooth workflow and patient-focused care.
Client and family support:
Provide support to clients and their families by educating them on safety, medication management, and other aspects of their care plan. Serve as a point of contact for clients and their families to address concerns and provide information. Supply Coordinator Ensure all medical and office supplies are adequately stocked at all times. Receive and organize supply deliveries, including unboxing, sorting, and shelving items in their designated areas. Monitor inventory levels and report low stock items to the appropriate department for reorder. Maintain a clean, organized, and easily accessible supply area. Verify incoming shipments for accuracy and condition of items. Required skills Strong organizational and time-management skills. Excellent verbal and written communication and interpersonal skills. Ability to work efficiently in a fast-paced environment and handle multiple tasks. Familiarity with home health regulations and documentation requirements.
