Job Description

Job Overview Wellspring Home Health is seeking an experienced and dependable Clinical Manager (DON) / Alternate Administrator to provide clinical leadership and oversight for our growing home health agency. The Clinical Manager (DON) will oversee clinical services, support quality patient care, coordinate clinical activities, and work closely with the Administrator to ensure effective agency operations. This position will initially be part-time/PRN , with the opportunity to transition to full-time as the agency grows and patient census, clinical workload, staffing needs, and operational responsibilities increase . The selected candidate will also serve as the Agency's Alternate Administrator and provide leadership and support when the Administrator is unavailable. Key Responsibilities Oversee the clinical management of the Agency's home health services. Oversee the implementation and evaluation of patient care services. Perform Start of Care (SOC) visits and other skilled nursing visits as needed. Complete and support accurate clinical assessments and patient care planning. Assist with appropriate staffing and assignment of clinical personnel. Review clinical records and documentation as needed. Participate in quality assurance and quality improvement activities. Assist with staff orientation, education, training, and performance oversight. Coordinate patient care with physicians, patients, families, and other members of the care team. Address patient and family concerns and assist with complaint resolution. Participate in case coordination and patient care planning. Work closely with the Administrator on clinical and Agency operations. Assist with regulatory and survey preparation activities. Help ensure that clinical services are provided according to Agency policies and procedures. Serve as Alternate Administrator when the Administrator is unavailable. Help ensure continuity of patient care and Agency operations when serving as Alternate Administrator. Perform other duties as assigned. Qualifications Current, active Texas Registered Nurse (RN) license . Minimum 2 years of nursing/clinical experience . At least 1 year of supervisory or administrative experience in home health or a related healthcare setting . Home health experience required. Comfortable performing Start of Care (SOC) visits and working in the field with patients. Strong clinical leadership, communication, and organizational skills. Ability to work independently and effectively with clinical staff, patients, families, and physicians. Ability to take on appropriate administrative responsibilities when serving as Alternate Administrator. Must successfully complete required background screening. Schedule & Growth Opportunity This position will initially be part-time , based on the Agency's current needs. As WellSpring Home Health grows, including increases in patient census, clinical workload, staffing needs, and overall operations, the position may increase in hours and transition to full-time . Part-Time/PRN → Increased Hours → Full-Time We are looking for someone who is interested in growing with the Agency and developing a long-term leadership role. Alternate Administrator Role The Clinical Manager (DON) will also serve as the Agency's Alternate Administrator . When the Administrator is unavailable, the Alternate Administrator will provide appropriate administrative leadership and support to help maintain continuity of Agency operations and patient care. Pay:
