Responsibilities:
1. Provide leadership and direction to the case management team, fostering a culture of collaboration, excellence, and patient-focused care. 2. Develop and implement effective case management programs, including utilization review, resource management, and patient advocacy. 3. Ensure compliance with all CMS regulations and other applicable laws and guidelines related to case management. 4. Collaborate with interdisciplinary teams to create and implement strategies aimed at reducing the length of stay and readmissions. 5. Conduct clinical documentation reviews to ensure accuracy, compliance, and optimal reimbursement. 6. Develop and manage departmental budgets, ensuring effective resource management. 7. Drive continuous quality improvement initiatives, setting and monitoring key performance indicators (KPIs) and implementing necessary changes to improve patient care and outcomes. 8. Foster staff development through training, mentorship, and performance evaluations. 9. Collaborate with senior leadership to align case management strategies with organizational goals and objectives. 10. Stay abreast of the latest developments, advancements, and trends in healthcare administration to ensure the organization's competitive edge.
Qualifications:
1. Bachelor's degree in Nursing, Healthcare Administration, or a related field. A Master's degree will be an added advantage. 2. A minimum of 3 years of experience in case management, with at least 2 years in a leadership role. 3. Demonstrated experience in care coordination, patient advocacy, length of stay management, interdisciplinary collaboration, and regulatory compliance. 4. Thorough understanding of CMS regulations, hospital operations, and clinical documentation review. 5. Proven track record in resource management, quality improvement, staff development, and healthcare administration. 6. Excellent leadership and team management skills, with the ability to motivate and inspire a high-performing team. 7. Strong analytical and problem-solving skills, with the ability to make informed decisions quickly. 8. Exceptional communication skills, both written and verbal. 9. Proficiency in using healthcare information systems and software. 10. Current state licensure as a Registered Nurse (RN) or equivalent, preferred. 11. Certified Case Manager (CCM) or equivalent certification, preferred.
