About Gameday Men's Health - Roanoke We are a men's health clinic focused on concierge‑level care in Testosterone Optimization, Peptides, Erectile Dysfunction, and Medical Weight Loss. Our patients are men 40-55 who want to feel like themselves again and are ready to invest in their health. We are growing and hiring a Patient Care Coordinator to own the patient journey, reduce cancellations, and help patients get life‑changing results. Role Mission This is a patient-facing sales and customer success role. The ideal candidate is an RN who enjoys building relationships, communicating with patients, driving retention, and helping patients identify additional services that may support their health goals. Responsibilities Responsible for maintaining and holding patients accountable through the patient journey from first visit through 12+ months, so patients: Stay on plan Feel supported and understood Renew and upgrade care instead of dropping off Patient onboarding & activation Call/text every new patient within 24 hours of starting treatment to welcome them and set expectations Make sure patients hit key "activation" milestones in their first 30-45 days (visits completed, labs reviewed, treatment plan clear) Confirm follow‑up appointments and lab visits Proactive check‑ins & support Maintain a daily list of "new" and "at‑risk" patients to contact Send check‑in texts/calls at scheduled intervals to review progress, answer questions, and troubleshoot issues before they become cancellations Coordinate with providers when clinical concerns come up Run a structured conversation focused on patient goals Track "saved" patients and follow up to ensure they stay on plan Ascension, reviews, and referrals Identify happy patients at 3-6 months and invite them to: Explore add‑on services (Peptides, ED, GLP‑1s, as clinically appropriate)Leave online reviews and testimonialsRefer friends and family Maintain, if not exceed monthly Key Performing Indicators (KPIs) Focus on monthly patient churn rate Add‑on service adoption and patient satisfaction scores You are part concierge, part coach, part closer. Experience Must be a Licensed Registered Nurse (RN) 2+ years in a patient‑facing, customer success, sales, or front‑office healthcare role High call volume experience (Preferred) Remote Experience (Preferred) Organized with strong follow‑through; you like lists, checkboxes, and CRMs Warm, empathetic communicator who can balance care with accountability Thrives in a fast‑paced, growing clinic environment Compensation & Schedule Performance bonus based on churn reduction, saved cancellations and additional services closed Full‑time, primarily Monday-Friday, with occasional evening call blocks as needed
