The Role As a Patient Outreach Coordinator, you will play a critical role in helping patients access the care they need while supporting the continued growth of our practice. You will be responsible for proactively connecting with patients, identifying opportunities for care, and helping patients move forward when they may otherwise fall through the cracks. You'll be an important connection between our practice and the communities, providers, organizations, and patients we serve. The right candidate will be someone who genuinely enjoys talking to people, building relationships, following up, and finding creative ways to connect patients with our services.Duties Include:
Proactively contact existing and prospective patients across the organization to schedule appointments and encourage patients to continue their recommended care. Run patient and insurance reports to identify opportunities for patient outreach and re-engagement. Follow up with patients who have outstanding recommendations, missed appointments, overdue follow-ups, or other opportunities for continued care. Make outbound calls to patients and provide a warm, professional, and personalized experience. Build and maintain positive relationships with referring providers and community partners. Develop and maintain organized outreach lists and track follow-up activities. Maintain accurate records of outreach activities, patient responses, referral opportunities, and follow-up needs. Work closely with Providers and Management to identify patient barriers and opportunities to improve patient access and engagement. Communicate patient feedback and outreach results to Management and Providers. Take daily direction from Providers and Management while also demonstrating initiative and the ability to work independently. Represent the practice professionally and positively at community events and outreach opportunities. Perform other duties as needed to support patient engagement, marketing, and the continued growth of the practice. What We're Looking For The ideal candidate will have: High school diploma or equivalent. 1+ year of experience in customer service, patient coordination, marketing, sales, community outreach, or a similar role. Excellent verbal and written communication skills. Confidence and comfort making outbound phone calls. Strong organizational skills and attention to detail. Ability to build genuine relationships with patients, providers, businesses, and community organizations. Ability to work independently, manage multiple priorities, and follow through on tasks. A positive, enthusiastic, and professional attitude. Experience in a medical or healthcare environment is preferred but not required. Referral development or community outreach experience is a plus.
