Position Summary The Wellness Coordinator supports the daily clinical operations of the community by assisting with resident assessments, monitoring resident health needs, maintaining compliance, coordinating services, and supporting caregivers and medication aides. This role works closely with the Wellness Director and leadership team to ensure residents receive exceptional care and services. Essential Job Duties Assist with resident assessments, reassessments, and service plans. Monitor resident health status and report concerns appropriately. Maintain accurate resident records and clinical documentation. Coordinate appointments, home health services, hospice, therapy, and physician visits. Communicate with families, physicians, pharmacies, and healthcare providers. Assist with resident admissions, transfers, and discharges. Review medication records and assist with medication management processes. Monitor resident incidents, falls, weights, and behavior changes. Assist with infection prevention and control monitoring. Participate in care conferences and interdisciplinary meetings. Assist with caregiver training and education. Maintain clinical supplies and equipment. Ensure compliance with state regulations and company policies. Provide direct resident care as needed. Perform other duties assigned by the Wellness Director or Executive Director. Qualifications Previous caregiving experience required. Previous clinical or healthcare experience required. Experience in Assisted Living, CBRF, RCAC, Long-Term Care, Home Health, or a related healthcare setting preferred. Strong organizational and communication skills. Ability to multitask and work in a fast-paced environment. Strong customer service and interpersonal skills. Proficient with electronic health records and Microsoft Office. Commitment to providing exceptional resident-centered care. Physical Requirements Ability to lift up to 50 pounds. Ability to stand, walk, bend, and assist residents throughout the shift. Ability to respond quickly during emergencies.
