Responsibilities Lifestyle Medicine program development & coordination Help support the growth and development of the practice's lifestyle medicine services including patient education, one-on-one support, group visits, and wellness programs. Utilize a Bible-based lifestyle medicine framework to support physical, emotional, and spiritual wellness. Patient Education & Individual Support Coordinate the Chronic Care Management program & care plans for eligible members offering one-on-one support (Medicare CCM). Group Support Coordinate group lifestyle medicine visits with Physicians / Providers facilitating the communication with patients and helping to create a positive experience. Other clinical and administrative tasks based on experience may include: Registered Dietitian / Nutrition Counseling, Medical Assisting, Lifestyle Medicine Health Coaching, Community / Church Wellness Event Support. Skills Exhibit effective communication both orally and in writing. Possess organizational and time management skills. Ability to build strong, trusting relationships with participants. Ability to positively resolve participant challenges and objections about their health and behaviors. Ability to provide empathy in difficult interpersonal situations. Eagerness to learn new skills, including problem solving training, exercise training and motivational interviewing. The Central Valley Located in California's San Joaquin Valley, Hanford, home to about 60,000 has a rich history, culture, and community.
