Sevita seeks a Program Management Home Manager to lead home- and community-based services in our Healthcare & Social Assistance programs. You will oversee daily operations, supervise care staff, and ensure person-centered support for individuals with diverse needs. Responsibilities include managing care plans, staffing, scheduling, compliance, and quality outcomes. You'll collaborate with families, clinicians, and community partners while fostering an inclusive, supportive team culture. Responsibilities Oversee home- and community-based care programs to ensure high-quality, person-centered services Supervise and coach direct care staff, providing feedback, training, and performance evaluations Develop, implement, and monitor individual care plans in collaboration with clinical teams and families Manage scheduling, staffing levels, and daily operations to meet regulatory and company standards Monitor program outcomes, documentation, and compliance with healthcare and safety regulations Coordinate with external partners, case managers, and healthcare providers to support client needs Track budgets, resources, and service utilization to support program goals and efficiency Respond to incidents and concerns, implementing corrective actions and continuous improvement initiatives Required Skills Program management in healthcare or social services Care coordination and service planning Staff supervision and performance management Knowledge of home- and community-based service regulations Electronic health record (EHR) and documentation systems Scheduling and workforce management tools Quality assurance and continuous improvement methods Budget tracking and basic financial management Risk management and incident reporting Microsoft Office or similar productivity software
