Job Description
Sevita is seeking a Program Management Program Supervisor to lead healthcare and social assistance programs that support individuals in community-based settings. This role oversees day-to-day program operations, ensures compliance with healthcare and social service standards, and manages program staff. You will develop and monitor program plans, track outcomes, coordinate with clinical and support teams, and drive continuous improvement. The Supervisor also mentors staff, supports training initiatives, and partners with families and community partners to deliver person-centered, quality care. Responsibilities Oversee daily operations of healthcare and social assistance programs in home and community-based settings Develop, implement, and monitor program plans, goals, and performance metrics Supervise, coach, and schedule program staff to ensure high-quality, person-centered services Ensure compliance with healthcare, licensing, and organizational policies and procedures Manage program budgets, resources, and documentation, including reports and records Coordinate with clinical, behavioral health, and support teams to integrate services Engage individuals, families, and community partners to support care plans and outcomes Identify and implement process improvements to enhance service quality and efficiency Required Skills Program management Project planning and scheduling Budget management Data analysis and reporting Risk management Process improvement Healthcare compliance knowledge Stakeholder communication Team leadership and supervision Electronic health record (EHR) systems
