DMH Field Supervisor Phoenix Home Care and Hospice - 3.6 Independence, MO Job Details $20 a year 1 hour ago Benefits On-the-job training Qualifications High school diploma or GED Full Job Description A Field Supervisor is responsible for ensuring high-quality care for clients in their homes, overseeing and supporting caregivers, and maintaining compliance with care plans and regulations.
DUTIES AND RESPONSIBILITIES
❖ Perform DMH new client opening including completion of all required paperwork and documentation. ❖ Responsible for completing 2 hours of On-The-Job-Training for each new PA. ❖ Provides DMH shift coverage as needed for personal care services, activities of daily living, and various household duties. ❖ Perform and schedule caregiver evaluations in the field. Encourage and acknowledge tasks completed satisfactory. Train and educate on tasks that need improvement after reviewing with the Program Manager. ❖ Perform quality assurance call/visits monthly with the client to monitor the client's status, the care we are providing, to monitor adherence to the Code of Ethical Behavior, dress code, documentation of client records and the work performance of the aide. ❖ Perform investigation/complaint visits after reviewing with Program Manager for action plan needed. This includes but is not limited to items being broke in the home, client issues with aide or concerns of non-compliance. ❖ Perform general office tasks as assigned by Program Manager. ❖ Follows established departmental policies, procedures, and objectives. ❖ Practices safety, environmental, and/or infection control measures. ❖ Performs miscellaneous job-related duties as assigned by Program Manager. #care2026 Requirements Reliable transportation and working telephone as required by the nature of the position. Education /
Licensure:
High school diploma or equivalent, some college or post- high school education preferred.
Experience:
Six months employment with Phoenix Home Care, be in good standing and have applied for no more than two internal applications/transfer requests within a 6 month period.
