Responsibilities:
Collaborate with healthcare professionals, residents, and families to coordinate comprehensive care plans that meet the unique needs of each individual Develop and maintain relationships with healthcare providers, hospitals, and other external agencies to ensure seamless transitions and continuity of care Conduct initial assessments, identify risks, and develop individualized care plans to mitigate potential health issues Educate residents, families, and staff on healthcare procedures, medication management, and wellness strategies Maintain accurate and detailed records of patient interactions, assessments, and care plans Participate in multidisciplinary care conferences and team meetings to ensure effective communication and collaboration Identify and report any concerns or issues related to resident health and well-being to the healthcare team and management If you are passionate about delivering exceptional healthcare services and enhancing the lives of others, please apply to join our team as a Healthcare Liaison .
