Overview We are seeking a dedicated and compassionate Direct Support Professional to provide essential assistance and care to individuals with diverse needs, including developmental disabilities, neurodevelopmental disorders, and aging-related conditions. Duties Assist residents with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support. Monitor residents' health status by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals. Support residents with medication administration and ensure compliance with care plans while adhering to HIPAA regulations for privacy and confidentiality. Facilitate engaging activities that promote cognitive function, social interaction, and emotional well-being. Prepare nutritious meals in accordance with dietary guidelines and assist residents with meal consumption as needed. Operate patient transfer equipment such as Hoyer lifts safely and effectively during resident transfers. Maintain a clean, safe environment by performing routine cleaning tasks and ensuring adherence to sanitation standards. Provide transportation assistance for medical appointments or community outings using appropriate safety protocols. Document care activities accurately in compliance with agency policies and healthcare regulations. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly desirable. Strong knowledge of patient observation techniques, vital signs measurement, and patient monitoring procedures. Proficiency in caregiving tasks such as assisting with ADLs, manual handling, and patient transfers using equipment like Hoyer lifts. Familiarity with medication administration protocols and medication management practices. Experience providing dementia care, Alzheimer's care, memory care, or hospice support is advantageous. Ability to communicate effectively with residents, families, and team members while respecting residents' rights and dignity. Knowledge of safety procedures including first aid, CPR certification, heavy lifting techniques, and emergency response protocols. Understanding of HIPAA compliance to ensure confidentiality of protected health information (PHI). Competence in meal preparation that meets nutritional standards for seniors or individuals with special dietary needs. Demonstrated ability to work independently or as part of a team in dynamic environments supporting individuals with disabilities or developmental challenges.
