As a Home Health Aide, you will be a trusted caregiver, supporting patients with daily activities, health monitoring, and emotional well-being while adhering to strict healthcare regulations and maintaining patient confidentiality. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately in electronic health record (EHR) or medical documentation systems. Administer medications and perform basic medical procedures such as catheter care, wound care, and assisting with end-of-life care when necessary. Support patients with meal preparation, cooking, laundry, cleaning, and other home care tasks to ensure a safe and comfortable environment. Document all patient interactions thoroughly while maintaining HIPAA compliance to protect patient confidentiality. Provide specialized dementia care, Alzheimer's care, memory care, and hospice support tailored to individual needs. Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and ensure optimal patient outcomes. Experience Prior experience working with geriatric patients or in assisted living facilities is highly desirable. Demonstrated knowledge of medical terminology, home healthcare regulations, and medical documentation practices. Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving. Proficiency in using EMR (Electronic Medical Records) systems and home health agency software for documentation and scheduling. Certifications in First Aid, Basic Life Support (BLS), CPR, and hospice or palliative care are preferred. Hands-on experience with patient transfer methods such as Hoyer lifts or other mobility aids is advantageous. Ability to perform heavy lifting safely and work independently in a home healthcare setting while maintaining high standards of patient care.
