Duties Assist residents with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patient health by taking vital signs, observing for changes, and reporting concerns promptly Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient privacy Support residents during transfers using manual handling techniques and equipment such as Hoyer lifts or patient transfer methods Prepare nutritious meals and assist with meal consumption while adhering to dietary restrictions and safety standards Maintain a clean, safe environment by performing regular cleaning tasks and ensuring infection control protocols are followed Document care activities accurately in resident records and update individualized care plans regularly Provide transportation assistance for appointments or community outings using personal or agency vehicles Engage residents in meaningful activities that promote social interaction, cognitive stimulation, and emotional well-being Follow all safety procedures including CPR, first aid, and emergency response protocols to ensure resident safety at all times Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient observation techniques and dementia or Alzheimer's care is essential Experience supporting individuals with autism or developmental disabilities is advantageous Familiarity with medication administration, vital signs monitoring, and care plan implementation required Ability to perform heavy lifting and manual handling safely while assisting residents with mobility or transfers Strong communication skills to effectively interact with residents, families, and team members Certification in CPR and first aid is required; additional certifications such as HIPAA training are a plus Demonstrated patience, empathy, and a positive attitude towards providing person-centered care Join us in making a positive impact!
