Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and independence. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, noting changes in health status and reporting them accurately Administer medications and perform basic medical tasks such as taking vital signs and catheter care under supervision Support patients with meal preparation, light housekeeping, laundry, and maintaining a clean living environment Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information securely and accurately Assist with patient transfers using proper methods such as Hoyer lifts or transfer belts, ensuring safety at all times Provide companionship and emotional support, especially for clients with dementia, Alzheimer's disease, or those receiving hospice or end-of-life care Skills Strong knowledge of medical terminology, patient care procedures, and home health care regulations Experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care Proficiency in using EMR/EHR systems for documentation and communication Ability to perform CPR and First Aid confidently; certification preferred but not required upon hire Excellent communication skills for effective client interaction and team collaboration Physical stamina for heavy lifting, patient transfers, and long shifts; familiarity with safety protocols such as HIPAA compliance and maintaining patient confidentiality Experience with hospice care, end-of-life support, or working in long-term care settings is highly valued Join us to provide compassionate home health care that enriches lives!
