Responsibilities Assist residents with Activities of Daily Living (ADLs), including personal care, grooming, dressing, and mobility support Monitor patient health by observing vital signs, administering medication as prescribed, and recording care activities in compliance with HIPAA regulations Support residents during meal preparation, feeding, and nutritional planning to promote healthy eating habits Operate patient transfer equipment such as Hoyer lifts and assist with manual handling to ensure safe mobility Implement individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, or neurodevelopmental disorder support Maintain a clean and safe environment by performing regular cleaning duties and adhering to infection control protocols Provide transportation assistance for appointments or community outings while ensuring safety and comfort Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred Knowledge of patient observation techniques and patient monitoring methods Familiarity with medication administration, vital signs measurement, first aid, CPR certification required or obtainable upon hire Experience working with individuals with autism, developmental disabilities, intellectual disabilities, or dementia is advantageous Ability to lift heavy objects safely using proper manual handling techniques and assist with patient transfers using equipment such as Hoyer lifts Strong communication skills to effectively interact with residents, families, and team members while respecting resident's rights Valid certifications in first aid and CPR; additional certifications in hospice care or geriatrics are a plus Requirements / Documents Needed Candidates selected for the position must be able to provide current: Valid Driver's License DMV Driving Record Vehicle Registration and Auto Insurance CPR Certification TB Clearance Physical Exam/Health Clearance Social Security Card for employment verification Join us in providing compassionate care that empowers individuals to live fulfilling lives.
