Ontario Rancho Cucamonga Rialto Fontana San Bernardino Loma Linda Redlands Job Summary Azure Home Health is seeking a compassionate and dependable Home Health Aide (HHA) to provide high-quality personal care services to patients in their homes under the supervision of a Registered Nurse (RN) or licensed therapist. The ideal candidate is caring, reliable, and committed to helping patients maintain their independence, dignity, and quality of life. Responsibilities Provide personal care including bathing, grooming, dressing, toileting, and oral hygiene. Assist patients with mobility, transfers, ambulation, and range-of-motion exercises. Help with meal preparation and feeding as directed in the care plan. Monitor and report changes in the patient's physical or emotional condition to the supervising nurse. Take and document vital signs as assigned. Assist with light housekeeping related to the patient's care. Maintain a safe, clean, and comfortable environment for the patient. Reinforce patient and family education provided by the clinical team. Complete accurate and timely documentation after each visit. Maintain patient confidentiality and comply with all agency policies and regulations. Attend required meetings and in-service training. Qualifications Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) certification (as required by state regulations). Current CPR/BLS certification. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel to patient homes within the assigned service area. Excellent communication and interpersonal skills. Ability to work independently while following the patient's care plan. Previous home health experience is preferred but not required. Physical Requirements Ability to lift, transfer, and assist patients safely. Ability to stand, walk, bend, and kneel for extended periods. Ability to perform physical tasks associated with patient care.
