We are seeking a dependable and compassionate Certified Home Health Aide (CHHA) to join our team and provide personal care and supportive services to patients in their homes. Job Summary The Certified Home Health Aide will provide hands-on personal care and assistance with activities of daily living under the direction and supervision of the appropriate healthcare professionals. The ideal candidate is compassionate, reliable, respectful, and able to work independently while maintaining excellent communication with patients, families, and the care team. Duties & Responsibilities Provide compassionate personal care and assistance to patients in their homes. Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and personal hygiene. Assist with safe transfers, ambulation, positioning, and mobility according to the patient's care plan. Provide assistance with meal preparation and feeding, when indicated. Assist patients with maintaining a clean, safe, and comfortable environment within the scope of the position. Observe and report changes in the patient's condition, behavior, appearance, or level of functioning to the supervising healthcare professional. Follow the patient's individualized plan of care and all agency policies and procedures. Maintain patient dignity, privacy, independence, and confidentiality at all times. Provide companionship and emotional support to patients and their families. Assist with routine household tasks that are directly related to the patient's care, as permitted by the care plan and applicable regulations. Use proper infection-control and safety practices. Maintain accurate and timely documentation of services provided and patient observations. Communicate effectively with nurses, therapists, supervisors, patients, and family members. Report safety concerns, incidents, changes in condition, or other significant observations promptly. Maintain professional boundaries and demonstrate respect for patients from diverse backgrounds. Perform other duties within the scope of the CHHA role as assigned. Qualifications Current Certified Home Health Aide (CHHA) certification as required by California regulations. Previous home health experience. Ability to provide compassionate, reliable, and respectful patient care. Ability to follow written and verbal instructions and work as part of a healthcare team. Strong communication and interpersonal skills. Reliable transportation and ability to travel to patients' homes in the assigned service area. Ability to maintain patient confidentiality and comply with applicable privacy requirements. Must be dependable, punctual, and professional.
