Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly to healthcare professionals. Provide companionship and emotional support to individuals with dementia, Alzheimer's disease, or other cognitive impairments, fostering a safe and engaging environment. Assist with mobility using equipment such as wheelchairs; ensure safety during transfers and ambulation. Perform light housekeeping duties including cleaning, laundry, and meal preparation to maintain a clean and healthy living space. Document medical records meticulously while adhering to HIPAA regulations to protect patient confidentiality. Qualifications Graduated from an accredited Certified Home Health Aide program Certified Home Health Aide Required High School Diploma or GED Driver's license required Proven experience in home health care, assisted living, nursing homes, or long-term care environments. Knowledge of medical terminology, medication administration protocols, catheter care, home health care practices, and dementia/memory care techniques. CPR (Cardiopulmonary Resuscitation) certification is required; additional certifications, such as HIPAA training, are preferred. Ability to work with individuals with disabilities or developmental challenges using patience and compassion. Willingness to travel throughout Los Angeles, San Bernardino, and Riverside Counties Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient information. Strong observational skills for patient monitoring and effective reporting of health status changes. Flexibility to work part-time schedules based on client needs; experience with home & community care is highly valued.
