Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Support medication reminders Provide companionship and emotional support, especially for clients with dementia or Alzheimer's disease, ensuring their safety and comfort Help with meal preparation, light cleaning, laundry, and maintaining a clean and safe living environment Document daily activities accurately on daily care log Assist with hospice or palliative care needs as required, respecting patient wishes and comfort measures Support clients in private home, community settings or assisted living facilities, ensuring consistency and quality of care Requirements Proven experience in home health care, senior care, long-term care, or assisted living environments Certification in CPR Certification such as hospice care or dementia training are a plus HCA Clearance TB Test Experience working with individuals with developmental disabilities or disabilities is highly desirable Strong communication skills to interact effectively with clients, families, and healthcare teams Ability to lift patients safely using Hoyer lifts or similar equipment when necessary Compassionate attitude with patience for working with individuals experiencing memory loss or cognitive decline Flexibility to work variable shifts or split shifts based on client needs Valid driver's license and reliable transportation preferred.
