Position Summary The Community Integration Specialist (CIS) / Medical Direct Support Professional (DSP) plays a vital role in supporting individuals with intellectual and developmental disabilities to live meaningful, self-directed lives. Under the direction of the Program Manager, the CIS/DSP provides direct support, teaching, coaching, and facilitation to help participants engage in meaningful activities aligned with their Person-Centered Plans (PCP). In addition to core direct support responsibilities, the CIS/DSP supports participants with specialized health care needs and performs authorized health-related procedures. The position works in coordination with UCP's licensed nursing staff and follows applicable physician orders, individualized health care plans, agency procedures, and regulatory requirements. Essential Functions Provide person-centered individual and group supervision and instruction that promotes independence, choice, dignity, inclusion, communication, social engagement, and progress toward PCP goals. Develop and facilitate individualized learning, social, pre-vocational, and community-based activities. Provide personal care and assistance with activities of daily living, including hygiene, toileting, dressing, eating, mobility, lifting, transferring, and positioning. Implement PCPs, individualized goals, and non-aversive behavior support strategies, as applicable. Ensure participant safety, including during vehicle loading/unloading and while following dietary orders. Perform delegated health care tasks and specialized procedures under authorized direction and in accordance with physician orders, agency policy, and applicable regulations. Assist with medication administration or support in accordance with authorized direction and agency requirements. Complete accurate and timely progress notes, incident reports, medical documentation, and other required records. Observe and promptly report changes in participant health, behavior, safety, or well-being. Protect participant rights, privacy, dignity, and confidentiality. Maintain regular and predictable attendance and participate in required meetings and training. Provide direct care support to other programs as needed. Required Education and Experience High school diploma or equivalent required; some college preferred. Experience supporting individuals with intellectual and developmental disabilities preferred. Licenses and Certifications Current CPR and First Aid certification, or the ability to obtain certification within the timeframe established by UCP. Conditions of Employment Successfully complete DOJ fingerprint clearance and all other required pre-employment screening requirements. Must complete 40 hours of required medical training within the first year of employment and demonstrate competency in delegated health care procedures under the direction of the LVN/Registered Nurse. Training may include, but is not limited to, G-tube, catheter, and colostomy care. Required Skills and Abilities Clear, professional, and respectful communication.
