In this vital role, you will provide compassionate psychosocial support and comprehensive case management to patients in their homes, ensuring they receive personalized care that promotes independence and well-being. Your expertise will help navigate complex healthcare needs, coordinate community resources, and support patients through challenging health journeys. Duties Conduct thorough patient assessments to evaluate psychosocial needs, medical history, and support systems. Develop individualized care plans aligned with clinical guidelines, social work practice standards, and patient goals. Coordinate community resources such as hospice services, mental health counseling, addiction treatment programs, and disability support. Provide counseling services including child & family counseling, behavioral therapy, crisis intervention, and clinical counseling tailored to each patient's circumstances. Collaborate with interdisciplinary teams—including nurses, physicians, therapists—to ensure seamless patient care and discharge planning. Maintain accurate medical documentation and records using EMR (Electronic Medical Records) systems while adhering to HIPAA regulations. Educate patients and families about disease management, medication adherence, and available community resources to foster independence and improve quality of life. Requirements Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law. Proven experience working with geriatric patients, individuals with disabilities, or those receiving hospice & palliative care. Strong knowledge of medical terminology, ICD coding, Medicare guidelines, and clinical ethical standards. Demonstrated skills in crisis management, motivational interviewing, diagnostic evaluation, and case management. Experience working with diverse cultural backgrounds and individuals with developmental disabilities or autism spectrum disorder preferred. Certification in Basic Life Support (BLS) and CPR required; additional certifications such as Applied Behavior Analysis or Cognitive Behavioral Therapy are a plus. Excellent communication skills for client interaction, group therapy facilitation, and clinical interprofessional collaboration.
