Join our compassionate hospice team as a Certified Home Health Aide (CHHA) , where you will play an essential role in providing comfort-focused, dignified care to patients with life-limiting illnesses in their homes, assisted living facilities, or other residential settings. As a Hospice CHHA, you will work under the supervision of a Registered Nurse and in collaboration with the interdisciplinary hospice team to assist patients with personal care and activities of daily living. This position is ideal for a dependable and compassionate caregiver who understands the importance of preserving dignity, comfort, safety, and quality of life during the end-of-life journey. Duties and Responsibilities Provide personal care according to the individualized hospice Plan of Care and under the supervision of the Registered Nurse. Assist patients with Activities of Daily Living (ADLs), including bathing, showering, bed baths, grooming, oral care, dressing, toileting, and incontinence care. Assist with repositioning, turning, transfers, ambulation, and mobility while following proper body mechanics and established safety procedures. Safely utilize assistive equipment, including wheelchairs, walkers, gait belts, and mechanical lifts such as Hoyer lifts when trained and authorized. Provide skin care and help maintain patient cleanliness and comfort to reduce the risk of skin breakdown. Assist with meal setup and feeding when indicated by the patient's Plan of Care, including following prescribed dietary and aspiration precautions. Change bed linens and perform light housekeeping tasks directly related to maintaining a clean, comfortable, and safe patient environment. Observe the patient's physical and emotional condition and promptly report changes, concerns, pain, skin changes, falls, changes in appetite, mobility, elimination, or level of responsiveness to the supervising nurse. Provide compassionate support to patients experiencing advanced illness, dementia, Alzheimer's disease, weakness, functional decline, or end-of-life changes. Offer respectful emotional support to patients and families while maintaining appropriate professional boundaries. Complete accurate and timely visit documentation in the agency's Electronic Medical Record/Electronic Health Record (EMR/EHR) system. Follow infection-control procedures, universal precautions, and agency safety policies. Maintain patient confidentiality and comply with HIPAA requirements at all times. Follow the hospice aide care plan and immediately communicate any inability to complete assigned care or any changes in the patient's needs to the supervising nurse. Participate in required training, competency evaluations, supervisory visits, and hospice team communication as directed. Perform only those duties permitted within the CHHA scope of practice and agency policy. Qualifications and Experience Current Certified Home Health Aide (CHHA) certification as required by applicable state regulations. Previous experience in hospice, home health, assisted living, skilled nursing, memory care, or other direct patient-care settings is preferred. Experience providing personal care to elderly, disabled, chronically ill, or terminally ill patients is highly desirable. Experience caring for patients with dementia or Alzheimer's disease is preferred. Understanding of hospice philosophy, comfort-focused care, patient dignity, and end-of-life care is preferred. Ability to safely assist with transfers, repositioning, ambulation, and use of patient-care equipment. Familiarity with electronic documentation and EMR/EHR systems is preferred. Ability to recognize and promptly report changes in a patient's condition to the supervising nurse. Strong communication, interpersonal, and documentation skills. Compassionate, dependable, respectful, and able to work independently while following the established Plan of Care. Ability to maintain patient privacy, confidentiality, and professional boundaries. Current CPR/BLS certification if required by agency policy or applicable regulations. Reliable transportation and ability to travel to patient locations as required.
