Duties and Responsibilities:

REQUIRED- Support individuals with maintaining a safe household by providing individualized support based on the Individual Support Plan. Accepts and works within established household routines. Supports the individual with laundry, shopping, meal planning and preparation, including household cleaning. Support with hygiene and daily health needs, including but not limited to: shower assistance, personal care, cleaning and medication. Adhere to filling out Medication Administration Form (MAR) & signing of chore lists. Welcome and respect the individual's friends and other visitors. Assist the individual in developing and maintaining positive relationships in the community. Write daily progress notes per your assigned shifts. Provide training of skills and learning opportunities to your assigned individual. Provide supervision and support for individuals that ensure all basic care needs are met in a timely and efficient manner. Accompany client to Urgent Care and Emergency Room visits and communicate results by entering the medical information in the daily progress notes. Support and follow through of any behavior/medical program set by the therapist/physician. Engage your assigned individual to be more participatory in their own life. Complete data collection for assigned individuals Attend monthly staff meetings, individual or group supervision, and in-service training sessions. Support the individual as they take their prescribed medication and maintain required documentation. Takes initiative in providing opportunities for normalization and personal development within the scope of their ISP. Complete 12 hours of in-service training annually. SLS Instructor Qualifications A positive outlook and the heart to make a difference. Ability to pass and maintain a LiveScan fingerprint clearance. Provide proof of valid CA driver's license and clean DMV printout, Must have a reliable vehicle, valid insurance, and vehicle registration. Applicants must have a minimum of one (1) year prior experience providing direct supervision and special services to persons with developmental and/or intellectual challenges. Applicants must provide a current Standard First Aid/CPR card. Applicants must have a high school diploma or GED. Experience in working with various medical conditions is highly desirable. Experience in implementing behavior modification program highly desirable. Complete any additional training as required on the job. A strong commitment to providing quality care and programming is essential.
