Job Description:
The Certified Home Health Aide (CHHA) provides direct personal care services to patients in the home setting under the supervision of a Registered Nurse or Therapist. This role supports patient comfort, safety, and daily living needs while following the established care plan. The CHHA plays a key role in maintaining a clean and safe environment and reporting changes in patient condition to the clinical team.
Reporting Relationship Clinical Supervisor Scope of Supervision None Essential Duties:
Provides personal care services in accordance with the established care plan, including bathing, grooming, oral hygiene, back rubs, shampoos, changed bed linen, skin care to prevent breakdown, brushing hair, brushing teeth and dressing Assists patients with toileting, transfers, positioning, and ambulation Performs range of motion (ROM) exercises as directed by the Registered Nurse or Therapist Monitors and records vital signs as directed and reports abnormalities to the Clinical Supervisor Assist patients with self-administration of medications as permitted Maintains cleanliness and organization of the patient's living environment Assists with light housekeeping tasks, including laundry and maintaining patient areas Prepares and provides simple meals, snacks, and fluids as directed Observes and reports changes in patient condition, including skin integrity and overall health status Documents care provided in accordance with agency guidelines and within required timeframes Uses appropriate personal protective equipment (PPE) and follows infection control protocols Education and/or
Experience:
