Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using appropriate patient transfer methods like Hoyer lifts or manual handling techniques. Monitor patients' vital signs, observe changes in health status, and report findings accurately to healthcare professionals. Administer medication as prescribed following HIPAA guidelines and care plans, ensuring proper documentation and patient safety. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support patients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and personalized engagement strategies. Maintain a clean, safe environment by performing light housekeeping tasks including cleaning patient areas and ensuring infection control standards are met. Provide transportation assistance for appointments or community outings while prioritizing safety during patient transfers and personal transportation needs. Document all care activities thoroughly, respecting residents' rights and confidentiality at all times in compliance with HIPAA regulations. Collaborate effectively with team members to develop and follow individualized care plans that support residents' health goals. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration. Experience providing dementia care, Alzheimer's care, memory care, or hospice support is advantageous. Knowledge of manual handling techniques such as Hoyer lifts and patient transfer methods is essential. Familiarity with caring for individuals with developmental disabilities or neurodevelopmental disorders enhances your ability to deliver personalized support. Strong communication skills for client interaction and team collaboration are required. Certification in CPR/First Aid is preferred; ongoing training will be provided for those interested in expanding their skills.
