We are seeking an organized and dependable PCA Program Manager to help coordinate services, support clients and providers, and keep program documentation accurate and current. Position Summary The PCA Program Manager oversees day-to-day coordination of our PCA services. This role serves as a point of contact for clients, families, providers, and other members of each client's support team. The ideal candidate is comfortable managing several cases at once, following up on details, and helping people access the support they need. Key Responsibilities Coordinate PCA services and monitor each client's ongoing needs and service delivery. Communicate regularly with clients, families, providers, and other members of the support team. Help develop, implement, and update service and individual care plans. Track provider documentation, follow up on missing records, and ensure monthly documentation is submitted on time. Maintain accurate client and provider records in Therap or other agency systems. Support provider onboarding, training, and ongoing communication about documentation and agency procedures. Attend service planning meetings and coordinate follow-up tasks. Help clients with service-related needs, including coordinating appointments, benefits, housing resources, and community supports when appropriate. Review files for completeness and help maintain compliance with agency and applicable program requirements. Identify service concerns, communicate them promptly, and work with leadership to resolve them. Complete occasional community visits or meetings as needed. Qualifications At least one year of experience in human services, disability services, case management, or a related field. Strong organizational, communication, and follow-up skills. Ability to manage multiple client and provider relationships while meeting deadlines. Careful attention to documentation and confidentiality. Comfort using electronic records systems; experience with Therap is preferred. Familiarity with Colorado Medicaid waiver services and person-centered planning is preferred. Reliable transportation for occasional off-site visits and meetings. Must be able to come into the office Monday-Fridays during working hours Education A Bachelor's Degree or higher in a related field is preferred. Relevant work experience will also be considered.
