Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity Monitor patients' health status by observing vital signs, patient behavior, and overall condition; report changes promptly to healthcare professionals Support medication reminder following prescribed care plans while adhering to HIPAA regulations to ensure patient privacy and confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Facilitate patient transfers using appropriate manual handling techniques and equipment or patient transfer methods to ensure safety for both clients and caregivers Maintain cleanliness of living spaces by performing light cleaning duties, laundry, and organizing personal belongings Provide companionship and emotional support, fostering a trusting relationship that encourages mental well-being Assist with transportation for medical appointments or community outings using safe client transportation methods Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is desirable Knowledge of patient observation techniques, dementia care (including Alzheimer's care), memory care, and hospice care practices Familiarity with care plans, medication administration, vital signs monitoring, and patient monitoring protocols Ability to perform heavy lifting and manual handling safely; experience working with individuals with disabilities or neurodevelopmental disorders is a plus Certification in CPR and First Aid; training in HIPAA compliance is desirable Strong communication skills for effective client interaction and team collaboration Valid driver's license or reliable transportation for assisting clients with personal transportation needs Join us in providing compassionate, professional care that truly makes a difference!
