Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts when necessary Monitor and observe patients' conditions by recording vital signs, noting changes in health status, and reporting findings accurately in Electronic Health Record (EHR) or Medical Record systems Administer medications following prescribed protocols while maintaining strict compliance with HIPAA regulations to ensure patient confidentiality Support clients with meal preparation and light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with medical procedures such as catheter care, wound care, and basic first aid or CPR in emergency situations Facilitate patient transfers using appropriate methods and equipment like patient lifts or Hoyer lifts to ensure safety during mobility assistance Qualifications Proven experience working as a Home Health Aide, caregiver, or in assisted living/nursing home settings with a strong understanding of home health care practices Knowledge of medical terminology, medication administration, vital signs monitoring, and home health regulations Experience working with individuals with developmental disabilities or geriatrics is highly desirable Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications related to hospice or end-of-life care are a plus Ability to communicate effectively with clients and team members while maintaining patient confidentiality under HIPAA guidelines Physical stamina for heavy lifting and long periods of standing or walking; ability to perform patient transfers safely Familiarity with EMR/EHR systems for accurate documentation of patient care activities Compassionate attitude combined with strong organizational skills to deliver high-quality home health assistance Join us in providing exceptional home healthcare that truly makes a difference!
