Live-in position including overnights and holidays with relief from PRN staff daily and designated weekends. This position requires a direct support live-in at the facility with provided room and board. Future candidates should be comfortable relocating to provide live-in services onsite daily throughout the year. This is a community-based residential facility in which you will be responsible for the well-being of the residents residing in the facility daily with relief from PRN staff daily, designated weekends and requested vacation.

Job purpose:

To provide leadership and direct care services to mental health adults throughout community integration, rehabilitation, skill building, health and safety supports, administration support, or personal care. Ensure a stable residential environment while relocating and living on the premise overnights. Duties & responsibilities: Direct Support Professional may engage in several rehabilitation or skill building activities with residents. In addition to the following job duties, the CRF Director and/or other management may request or remove duties at anytime during employment.

• Perform active engagement activities, skill building, and rehabilitation with residents.
• Prioritize the health and safety of all residents. Maintain comprehensive and accurate documentation, including progress notes, Medication Administration Records (MARs), and Major Unusual Incident (MUI) reports, in accordance with regulatory requirements.
• Provide excellent customer service with support team and case managers.
• Complete accurate notes and incident reports as required by regulations and policies.
• Provide meal preparation during mealtimes according to menu guidelines, ensuring nutritional standards are met and dietary requirements are followed. This includes inventory management of the food pantry, grocery shopping coordination, and maintaining a high standard of kitchen cleanliness and food safety.
• Provide support and assistances to team members.
• Promote a safe, clean, and orderly work environment. Follow all written protocols, policies and procedures regarding safety and risk management. Report any concerns of unsafe conditions immediately to the proper personnel.
• Communicate with PC through weekly team meetings and maintain accurate meeting minutes.
• Ensure MARs and medical books are updates daily.
• Administer medications in accordance with established protocols and promptly report any medication errors to management.
• Maintain a fully stock food pantry and household supplies
• Maintain a clean worksite, personal space, and including daily cleaning and bedroom checks.

Minimum level of

Education:

High School Diploma/ G.E.D. are required and years of experience. Maintaining accurate CPR and First Aid Certification. Must have a negative PPD test completed and maintain all other certifications that may be asked by Program Coordinator and/or Quality Assurance Coordinator. A satisfactory criminal background check and CPS check required.

Skills required:

This job requires several skills but not limited to: Clear and effective verbal and written communication skills. Must be able to use standard Microsoft Office applications. Must be efficient in computer skills and maintaining notes. Use Google email, calendar, contacts, and drives. Food preparation, storage, and cooking skills. Proper sanitizing, cleaning, and janitorial skills. First Aid, CPR, and Emergency Preparedness.
