In this vital role, you will provide essential support to clients in their homes or community settings, ensuring their safety, comfort, and well-being. Your proactive approach and empathetic nature will help promote independence and quality of life for those you serve. This position offers the opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs such as blood pressure, pulse, and temperature accurately Support medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations and medical documentation standards Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely and efficiently Use assistive devices such as Hoyer lifts or catheters to ensure safe patient transfers and hygiene management Prepare nutritious meals aligned with dietary needs and assist with light housekeeping tasks like cleaning and laundry to maintain a safe environment Collaborate with nursing staff and healthcare professionals to coordinate patient care plans in home or community settings Support patients during hospice care by providing comfort measures and emotional support in accordance with care plans Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments Knowledge of medical terminology, medication administration protocols, and vital signs monitoring Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus Ability to work independently while following detailed care plans and medical instructions Experience working with individuals with developmental disabilities or disabilities is highly desirable Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Compassionate attitude with excellent patient observation skills and strong communication abilities Valid driver's license and reliable transportation preferred for community-based visits Join us in delivering exceptional home health care that transforms lives!
