Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates to track health status accurately Administer medications following prescribed schedules and document medical records in Electronic Health Record (EHR) systems while maintaining HIPAA compliance Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Support patients during assisted living or long-term care settings by helping with meal preparation, light cleaning, laundry, and maintaining a safe environment Assist with medical procedures such as catheter care and wound management under the supervision of nursing staff or physicians Document all patient interactions thoroughly in medical documentation systems and communicate any changes in condition promptly to healthcare teams Use medical terminology confidently to ensure clear communication within healthcare teams and accurate recordkeeping Support hospice care patients by providing compassionate end-of-life assistance aligned with their wishes and comfort needs Experience Prior experience working in home health or community-based caregiving environments preferred Experience in nursing homes or assisted living facilities is highly valued Demonstrated ability to work effectively with individuals with developmental disabilities or dementia-related conditions Knowledge of medical documentation practices and familiarity with EMR (Electronic Medical Records) or EHR systems required Certification in CPR (Cardiopulmonary Resuscitation) is essential; additional certifications such as medication administration or hospice care are a plus Long-term care experience that includes working with seniors or individuals with disabilities enhances your candidacy Strong understanding of medical terminology and patient safety protocols necessary for providing high-quality home health services Join us to bring energy, compassion, and expertise into every patient interaction.
