Overview We are actively seeking dedicated and compassionate Home Health Aide (HHA) professionals to join our team immediately. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient health status by observing vital signs, managing medication administration, and recording medical information accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavioral support. Support patients with complex medical needs such as catheter care, end-of-life assistance, and hospice care while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met comprehensively. Demonstrate proficiency in medical terminology and basic life support (BLS) procedures such as CPR to respond effectively during emergencies. Support clients in activities that promote independence and well-being within their homes or community settings. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly desirable. Demonstrated experience caring for geriatric patients or individuals with disabilities, including developmental disabilities or chronic conditions like dementia or Alzheimer's disease. Familiarity with home health documentation systems such as EMR/EHR platforms and medical records management. Proven ability to perform patient transfers safely using various methods including Hoyer lifts or other transfer techniques. Experience providing long-term care or live-in employment that involves comprehensive patient personal care assistance. Knowledge of home health regulations, medication management protocols, and safety procedures such as first aid and basic life support (BLS). Ability to handle physical tasks such as heavy lifting while maintaining patient safety standards.
