Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and document patient vital signs and overall health status. Provide companionship and emotional support to clients, fostering a positive environment. Administer medications as prescribed and ensure proper medication management. Assist with mobility and transfers using equipment such as Hoyer lifts when necessary. Perform light housekeeping duties including cleaning, laundry, and meal preparation to maintain a safe living environment. Support clients with Alzheimer's care, dementia care, and memory care techniques. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with healthcare professionals to develop and implement individualized care plans. Provide care for individuals with developmental disabilities or special needs. Requirements Previous experience in home health care, senior care, or long-term care is preferred. Knowledge of medical terminology and familiarity with EMR/EHR systems is advantageous. CPR certification is required; additional certifications in catheter care or hospice care are a plus. Strong observational skills to monitor changes in patient conditions effectively. Ability to work compassionately with individuals suffering from various conditions including Alzheimer's and dementia. Excellent communication skills for effective interaction with clients, families, and healthcare teams. Must be able to perform physical tasks associated with caregiving such as lifting and transferring patients safely. A valid driver's license may be required depending on the job location.
