Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by observing vital signs and recognizing changes in condition; accurately document medical information using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while maintaining compliance with HIPAA regulations for patient confidentiality Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals Prepare nutritious meals tailored to dietary needs and assist with feeding when required; perform light cleaning and laundry to maintain a safe and hygienic environment Maintain detailed medical records and documentation of patient care activities to ensure continuity of care and compliance with healthcare standards Work effectively within assisted living or long-term care settings, demonstrating patience and compassion while supporting clients' physical and emotional needs Experience Prior experience working in home health, nursing homes, or assisted living facilities is highly preferred Demonstrated ability to work with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease Familiarity with medical terminology, vital signs measurement, medication administration, and medical documentation practices Experience using EMR/EHR systems for accurate record keeping and communication among healthcare teams Certification in CPR is required; additional certifications such as hospice care training or catheter care are advantageous Proven ability to handle sensitive situations with empathy while maintaining professionalism and confidentiality under HIPAA guidelines Long-term care experience including home & community care environments is highly valued Join us in providing compassionate, dedicated care that transforms lives!
