In this vital role, you will provide personalized care to promote independence, comfort, and well-being for clients across diverse settings. Your active engagement and attention to detail will ensure that each person receives the highest quality of care in a safe, respectful environment. This position offers an opportunity to work closely with patients, families, and healthcare teams to deliver comprehensive home health services that enhance quality of life. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to support their independence. Monitor and record vital signs including blood pressure, pulse, respiration, and temperature to track health status accurately. Administer medications following prescribed schedules and document medication administration in medical records or electronic health record (EHR) systems. Support patients with mobility needs using equipment like Hoyer lifts and assist with transfers safely. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management. Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe and comfortable living environment. Document all patient interactions and medical information in compliance with HIPAA regulations using EMR (Electronic Medical Records) or EHR systems. Assist with medical procedures such as catheter care or hospice-related activities under supervision or direction from healthcare professionals. Support patients during emergencies by implementing CPR or other first aid measures when necessary. Collaborate with healthcare teams to ensure continuity of care and communicate any changes in patient condition promptly. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to perform patient observation and accurately record medical data. Experience working with individuals with developmental disabilities or dementia is advantageous. Knowledge of medical terminology relevant to home health care practices. Certification in CPR is required; additional certifications such as medication administration or hospice care are a plus. Familiarity with EMR/EHR systems for documentation purposes is desirable. Long-term care experience and working with patients requiring memory or hospice care will strengthen your application.
