This vital role involves assisting individuals with daily activities, ensuring their safety, and promoting independence within the comfort of their homes. As a Home Health Aide, you will play a crucial part in enhancing quality of life for patients while adhering to home healthcare regulations and maintaining the highest standards of patient confidentiality. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, noting changes in health status and reporting findings accurately Administer medications and perform basic medical tasks such as taking vital signs and catheter care under supervision Support patients with mobility transfer methods using equipment like Hoyer lifts or other assistive devices Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA standards Provide companionship and emotional support, especially for clients with dementia, Alzheimer's disease, or those receiving hospice or end-of-life care Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Skills Strong patient observation skills with the ability to recognize changes in health or behavior Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly desirable Knowledge of medical terminology, medication administration, and vital signs measurement Experience working with individuals with developmental disabilities, geriatrics, dementia, or memory care needs Familiarity with home health regulations, HIPAA compliance, and medical documentation procedures Ability to perform patient transfer methods safely using equipment like Hoyer lifts or other assistive devices Certification in First Aid and Basic Life Support (BLS), including CPR training is required or preferred Excellent communication skills for effective client interaction and collaboration with healthcare teams Physical stamina for heavy lifting and assisting patients with mobility challenges Compassionate caregiving approach combined with professionalism and respect for patient confidentiality Join our team to provide compassionate home health care that truly makes a difference!
