Job Overview Join our compassionate team as a Home Health Aide-Pediatric, where your dedication will make a meaningful difference in the lives of children with special healthcare needs. In this vital role, you will provide personalized in-home care tailored to each child's unique requirements, promoting comfort, safety, and independence. Your energetic and caring approach will help children thrive in their familiar environments while supporting their families every step of the way. This position offers the opportunity to work closely with families and healthcare teams to deliver exceptional patient-centered care that fosters growth and well-being. Responsibilities Assist children with Activities of Daily Living (ADLs), including bathing, dressing, grooming, feeding, and mobility support using tools like Hoyer lifts when necessary. Monitor and document vital signs such as temperature, pulse, respiration, and blood pressure accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications and manage medical treatments following physician orders while maintaining strict adherence to HIPAA regulations for patient privacy. Observe and report on patient condition changes, including symptoms related to developmental disabilities, dementia, Alzheimer's disease, or other chronic conditions like hospice or memory care needs. Perform specialized care tasks such as catheter care, wound dressing assistance, and supporting patients with dementia or Alzheimer's in memory care settings. Support families by preparing nutritious meals, assisting with light housekeeping tasks like cleaning and laundry, and ensuring a safe home environment. Utilize medical terminology knowledge to accurately document medical records and communicate effectively with healthcare professionals. Employ skills in working with individuals with disabilities across various settings including assisted living facilities, nursing homes, or home environments. Demonstrate proficiency in using home health equipment such as Hoyer lifts and vital signs monitors while ensuring patient safety at all times. Maintain compliance with all regulatory standards including HIPAA and ensure confidentiality of medical documentation and records. Skills Strong patient observation skills to detect changes in health status promptly. Extensive experience working in home health care environments or long-term care settings such as nursing homes or assisted living facilities. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Knowledge of assisting with ADLs and providing compassionate caregiving tailored to pediatric patients' needs. Experience working with individuals who have developmental disabilities, dementia, Alzheimer's disease or require hospice or memory care services. Ability to perform medical procedures such as medication administration, catheter care, and vital signs monitoring confidently. Certification in CPR is required; additional certifications like hospice care training or specialized pediatric caregiving are a plus. Capable of working independently in patients' homes while coordinating effectively with healthcare teams. Excellent communication skills for collaborating with families, medical professionals, and caregivers while maintaining confidentiality under HIPAA guidelines. Willingness to perform light housekeeping duties including cleaning and laundry to support overall patient well-being.
