As a vital member of our healthcare team, you will deliver personalized support that promotes independence, safety, and well-being. Your energetic approach and attention to detail will ensure clients receive the highest quality of care in their own homes, fostering comfort and dignity every step of the way. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities and medical needs. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and perform catheter care following home healthcare regulations and medical protocols Support clients with dementia, Alzheimer's, memory care, and end-of-life needs with patience and compassion Use proper patient transfer methods such as Hoyer lifts or other assistive devices to ensure safety during mobility assistance Maintain patient confidentiality in compliance with HIPAA regulations while handling medical records and sensitive information Provide light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment Respond promptly to client emergencies with first aid or basic life support (BLS) techniques when necessary Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and report changes in condition Experience Proven experience working as a Home Health Aide or in assisted living facilities with geriatric or disabled populations Prior experience in nursing homes or hospice care environments is highly desirable Familiarity with home health care assistance, long-term care settings, and community-based services Knowledge of medical terminology, medication administration procedures, and vital signs monitoring Experience working with individuals with developmental disabilities or dementia is preferred Demonstrated ability to handle heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Working knowledge of EMR/EHR systems for documentation purposes Certification as a Home Health Aide from an accredited program is required; CPR/BLS certification is a plus Join us in delivering energetic, personalized care that transforms lives!
