In this vital role, you will provide essential support to individuals with diverse needs, including elderly patients, those with developmental disabilities, and individuals receiving hospice or dementia care. Your active engagement will promote independence, comfort, and dignity while ensuring adherence to healthcare standards and regulations. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to promote independence and comfort Monitor patient health by observing vital signs, documenting medical information accurately in Electronic Health Record (EHR) systems, and reporting changes promptly to healthcare professionals Support medication administration following prescribed protocols while maintaining strict compliance with HIPAA regulations to ensure patient confidentiality Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with medical procedures such as catheter care, wound care, and end-of-life support in accordance with home healthcare regulations and best practices Transfer patients safely using patient transfer methods like Hoyer lifts or other assistive devices; perform heavy lifting when necessary while prioritizing safety for both patient and caregiver Prepare nutritious meals, assist with feeding when needed, and maintain a clean and safe environment through light housekeeping tasks like laundry and cleaning Qualifications Proven experience working as a Home Health Aide or in assisted living/nursing home settings with geriatric or disabled populations Knowledge of medical terminology, medication administration, vital signs measurement, and basic life support (BLS) or CPR certification Familiarity with EMR/EHR systems for accurate documentation of patient care activities Strong understanding of home health care assistance regulations, HIPAA compliance, and patient confidentiality standards Ability to perform caregiving duties such as assisting with ADLs, transferring patients safely using various methods including Hoyer lifts, and providing end-of-life or hospice care when required Excellent communication skills for effective client interaction and team collaboration Physical stamina for activities involving heavy lifting, prolonged standing or walking, and assisting with mobility needs Experience working with individuals with disabilities or developmental challenges is highly preferred; prior home health agency experience is a plus Join us in delivering compassionate care that empowers our clients to live comfortably at home.
