Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure their comfort and hygiene. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly. Support medication administration following prescribed instructions while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral support. Operate Electronic Health Record (EHR) systems to document patient interactions accurately and maintain comprehensive medical records. Assist with mobility and transfers using patient transfer methods such as Hoyer lifts or other assistive devices to ensure safety during movement. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Support end-of-life care with compassion and respect, ensuring comfort and dignity for terminally ill patients in hospice settings. Requirements Proven experience working as a Home Health Aide or in assisted living facilities; experience with geriatrics or long-term care preferred. Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) including CPR certification. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Ability to work independently while maintaining excellent communication with clients and healthcare teams. Physical ability to lift heavy loads safely and perform patient transfers using appropriate techniques such as patient transfer methods or Hoyer lifts. Understanding of home health care regulations, HIPAA compliance, and patient confidentiality standards. Experience working with individuals with disabilities or developmental disabilities is highly desirable.
