Valid Driver's License Professional License (HHA) (Must be FDoE approved) Free of Communicable Disease Statement- Dated/Signed within last 6 months CPR Card (Only American Heart Association, American Red Cross, OR American Safety and Health Institute HIV Certificate
- 1 hour Alzheimer's/Dementia
- 2 or more hours approved by DOEA only Assistance with Self Medications or Self Administration of Medications 2 hrs Auto registration and auto insurance if providing transportation Previous Employment/Contract history for the last five years Professional Liability Insurance
