Job Description Summary ‎ ‎ How will you make an impact & Requirements ‎
Duties of Position :
The Medical Social Worker provides medical social services to clients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing or other appropriate supervisor.

Assesses the client's potential to cope with his/her social and health problem Acts as consultant to the members of the health team; helps the team understand the social, emotional and environmental factors related to the client's health problem Helps clients to utilize the resources of their families and the community. Provides rehabilitative and supportive cas Assists clients and their families cope with personal and environmental difficulties which might predispose them to illness or might interfere with their ability to obtain maximum benefits. Assists in the client's admission process to the Agency by performing the initial evaluation and assessment of the client's psychosocial status as well as an assessment of the client's home and family to identify socioeconomic and emotional factors that will affect the treatment plan. Assists in development and implementation of the interdisciplinary client care plan as it pertains to medical social work. Observes, records and reports changes in the client's emotional and social factors that affect the client's illness and his/her need for care as well as his/her response to treatment. Consults with the attending physician concerning alteration of the plan of treatm Enters clinical notes into EMR within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and progress notes and other clinical record forms outlining the services rendered. Evaluates the client's and family's responses to, and effectiveness of, the medical social work intervention. Confirms, on a weekly basis, the scheduling of visits with the Director of Nursing to facilitate coordination of other staff visits. Participates in Agency activities and committees that include staff development and in-service education when appropriate. Supervises the social work assistant once mont Participates in discharge plan Assumes responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and reading. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
Job Conditions :
Position requires the ability to communicate well orally and in writing. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds and to lift and/or carry up to 30 pounds. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition; to perform and demonstrate client care; and, to assess a client's home that may not be wheelchair accessible.
Qualifications:
Masters Degree from a School of Social Work approved by the Council of Social Work Education. Two years' experience with at least one year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.) This job description is not intended to be all-inclusive. Compensation Range:
$18.00 to $27.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
