Overview Join our compassionate healthcare team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of patients and their families. In this vital role, you will provide personalized in-home care, supporting individuals with diverse needs including seniors, those with disabilities, or those recovering from illness. Your energetic and positive approach will help promote independence, comfort, and well-being for every patient you serve. This position offers the opportunity to work closely with healthcare professionals and gain valuable experience in home health and long-term care environments. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure patient stability Support medication administration following proper protocols and documentation standards while maintaining HIPAA compliance Observe and report changes in patient condition promptly to healthcare teams to facilitate timely interventions Provide companionship and emotional support to reduce feelings of isolation, especially for individuals with dementia or Alzheimer's disease Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Document all care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to maintain comprehensive medical records Skills Proven experience working with individuals with developmental disabilities, dementia, or in hospice care settings Strong knowledge of medical terminology and procedures related to patient care and safety standards Ability to perform patient observation effectively and recognize signs requiring urgent attention Experience operating medical equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other vital sign monitoring devices Familiarity with home & community care practices, including working with EMR/EHR systems for documentation Excellent communication skills to collaborate seamlessly with healthcare teams and family members Valid CPR certification along with training in catheter care, medication administration, and hospice or memory care preferred Join us in delivering exceptional home health services that empower individuals to live comfortably and safely at home.
