Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide high-quality, personalized care to clients in their homes. This vital role involves assisting individuals with daily activities, ensuring their safety, comfort, and well-being. As a Home Health Aide, you will play a crucial part in supporting clients' independence while maintaining strict adherence to healthcare regulations and confidentiality standards. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' conditions, noting any changes or concerns related to health status or behavior Administer medications and manage medical devices such as catheters under proper protocols Record patient information accurately using Electronic Health Record (EHR) systems and maintain detailed medical documentation Support clients with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable environment Implement patient transfer methods safely using equipment like Hoyer lifts or transfer belts while following proper techniques Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those requiring hospice or end-of-life care Requirements Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) including CPR certification Familiarity with home healthcare regulations such as HIPAA and maintaining patient confidentiality Ability to perform heavy lifting when necessary and work with mobility aids like Hoyer lifts or patient transfer devices Strong communication skills for effective client interaction and documentation in EMR/EHR systems Experience working with geriatric patients, individuals with disabilities or developmental challenges, dementia care, and memory care is highly desirable Valid certification in first aid and CPR; additional certifications in hospice or end-of-life care are a plus Ability to work independently while following all safety protocols and maintaining professionalism at all times Join our team to deliver compassionate home health care that empowers clients to live safely and comfortably in their own homes.
